This form allows managers to fill in most fields on their computer using Adobe Reader and to save their changes.  For the sake of legibility, we are asking managers to complete as much of the form as possible electronically before printing both pages and adding all necessary signatures.  Once completed, the printed form would be submitted to your League Representative.  PLEASE DO NOT ATTEMPT TO SUBMIT THIS FORM VIA E-MAIL TO YOUR LOCAL ASSOCIATION, LEAGUE REPRESENTATIVE, THE STATE OFFICE OR ANY NDASA OFFICER AS IT WILL NOT BE ACCEPTED.
